US says blunders led to botched September strike in Syria

A U.S. military investigation has concluded a series of "unintentional human errors" led to a coalition airstrike on Sept. 17 that killed at least 15 fighters* aligned with the Syrian government, believed at the time to be from ISIS.

Brigadier General Richard Coe, the investigating officer, told reporters that the errors ranged from misidentification of targets to "group think" during intelligence development and communication errors in a hotline with Russia.
